You need to complete the Application for Driver’s License and provide legal proof of name change. You also need to take photo. No fee is charged for this procedure.

Changing Address on Driver License in State of New Hampshire: You need to notify DMV within 10 days about your change of address. You can visit DMV substation in person to carry out the formalities. You can even complete the Record Change Request and mail it to DMV along with the legal proof of address change. No fee is charged for this procedure.
